Methods| This prospective cohort study was performed on a medical unit of the University of
Colorado Hospital, Aurora, a 412-bed academic tertiary care hospital, from October 1, 2012,
through March 31, 2013. Approval was obtained from the Colorado Multiple Institutional
Review Board and the University of Colorado Hospital Research Review Committee.
