We read with interest the article by Ary Serpa Neto and colleagues, 1 which reported that
high driving pressure during intraoperative ventilation and a change in the level of positive
end expiratory pressure (PEEP) that results in an increase in driving pressure are
associated with more pulmonary complications after surgery. An increase in the level of
PEEP could theoretically lead to a decrease in driving pressure when tidal volume is
